A great comprehensive guide. Reader friendly. Treatment approaches are comprehensive backed up by the latest evidence.
They even discussed the length of treatment recommended something that has been somewhat neglected in other sources.
If it had Q&A I would have given it a 5 otherwise it should be 4.5 but there were no half stars.
